If you click outside of the content area while an image toolbar is open the image is highlighted rather than losing focus like it does when you click elsewhere in the content area. Chrome 46/OS X 10.11

There's a similar issue with galleries, but a little worse in that you can't refocus the gallery to access the toolbar without clicking elsewhere in the content area first:
