Appointment Start date is missing when tried to view the past appointment #135

Description
STR:

Select Appointment List option from the Manage Appointments tab
Select any past appointment and select Edit option
Notice that the Start date is not displayed in the date text field for past appointment

Note: Start date is not displayed for Missed, Past and Cancelled appointment.
