Describe the bug
Covers with same name ended up using earlier upload image

To Reproduce

Have two different images with same name (i.e. 'clean.jpg')
Upload first as cover to issue 1
Upload second as cover to issue 2
For issue 2 you end up with cover from issue 1

Expected behavior
Covers displayed correctly
