Implementing some input elements

Currently, i'm working on implementing some input elements such as Radio Box, Check Box and a special phone number input, which aims to represent a set of numbers in a human-readable format

@jerichofs Because of the fact that this issue contains phone number input as a primary target I set its story points(estimates) to 21 just like my Facebook login implementation. Don't underestimate phone input :-) .
I'm moving it to backlog, but when you start working with it it should be moved to progress
