Set feedback and review forms on set pages

Opening this discussion to continue conversation about player feedback on sets and revisions from the discord #retroachievements chat. Among ideas discussed:

1. Instituting a player feedback form a la the Google Form devised by user Fridge for their personal use, embedded on set page, allowing for aggregated player feedback and actual data points about the set
2. Including the player's progress on the set and their global stats (ranking, point total, games played, average completion, etc) at the time of submission for additional context of the review
3. Some mechanism to allow players who have not yet played the set, but have player the game outside of RA to also give some kind of feedback

In general, I think the best approach would be to allow as many types of people to give feedback as possible, and then the devs would have many points to filter down by to focus in on what is important to them. Having this stored on the DB would allow for historical analysis as well.
