Any mekanism machine energy input default to all sides - block extender (no config change) - energy pipe - energy source

On placement all items connect. On exit + reload connection is broken and reconnects when reconfiguring block extender (change any value and change it back and it connects).

Suspect mekanism machine not readable by block extender on load, but ok when running?

In my main world. I had translocators pop off refined storage block extenders on world load only where they were connected to mekanism machines. Tried to reproduce with as few mods as possible.

As follow-up tried the Proxy mod that adds a "proxy" block similar to block extender and it functions normally with Mekanism.
