Adding a reusable block more than one time to the editor results in a duplication of the block's settings in the sidebar and toolbar.

To Reproduce
Steps to reproduce the behavior:

  1. Create a reusable block
  2. Add it more than one time to the editor
  3. Edit onde of the blocks
  4. See the settings duplication in the sidebar and toolbar

Desktop, Gutenberg 4.2.0

Can reproduce, seems to happen with any block type within the reusable block:

I'm unable to reproduce this in WordPress 4.9.8 + Gutenberg 4.5.1 and WordPress 5.0 RC.
