Add lists of long-running request URLs to the XLT report.

there should be a list of long-running URLs per request type (similar to the 10 first unique URLs per requests type we have today)
the grouping of request types should follow the request merge rules
list only URLs that ran longer than a certain time (should be configurable, default may be 3000 ms)
limit the number of listed URLs per request type (may be configurable, default may be 10)
