align form styles throughout the app

This is a placeholder ticket for @bradleyrichter and I to sit down and make sure all form elements (input, select, button) are styled consistently throughout the app.

One of the main problems right now is the appropriate styles require a very nested element (something like settingsList > settingsItem > input) but we should pull them out into a simple class so they can be used app wide.

I created two new specific fixes and assigned them to myself for 0.12.8.
