Update account registration requirements

Some improvements can probably be made to the new accounts page. For example:

• include the new account holder's actual name
• include a slot for a "sponsoring" account (ie. someone with an existing account who can vouch for the new member)

This kind of goes towards who the sponsoring account is, but might be worth just tracking the content provided in the signup form of "How did you hear about us" for non-approved accounts just to be able to get more context, especially if the sponsoring account is like from 4 years ago.
