Index Exchange Bid Adapter: coppa support, dealid & ttl field updates (…

* set bidderRequestId to be a string in r.id

* set coppa value in r.reqs.coppa

* added support for seat[].bid[].exp ttl value

* read dealid from openrtb first, then fallback to ext

* use utils.isInteger
