table of contents cleanups #34

The numbering/table of contents for the spec has a number of quirks.

All the requirements are listed under "WorkflowStep" but should be their own section
"SoftwarePackage" should be under "SoftwareRequirement"
the "Workflow" spec includes a bunch of stuff from CommandLineTool that doesn't belong there
