Often, when a DocBook procedure is mapped to AsciiDoc, the steps in the numbered list are too deeply nested. For example, in the Fabric_Guide/get_start.xml file, there is an example of a procedure that gets mapped to a triply-nested numbered list (for no good reason!).

Effectively duplicates issue #16. The commit for #16 also fixes this issue.
