I came across this while testing NVICO for a separate bug and it might explain some of the weirdness reported by a few users.
Specifically, some NPCs do not have face mods chosen in the autosave, which is not a bug when they point to vanilla (e.g. Skyrim.esm). However, when a new mod provides a choice that wasn't available before, such as NVICO with ID 013382:Skyrim.esm (just one example), the mod gets flipped during startup but the plugin is then overwritten by profile restore. This can leave a discrepancy between the two settings that is not the result of user error.
