Describe the bug
the multiple choice and single choice questions accepts redundant answers where the choice must be unique

To Reproduce
Steps to reproduce the behavior:

Go to create form
change the question type to single or multiple choice
add multiple options with the same value
hit create form
the form is created successfully

Expected behavior
validation error the options must be unique for the form question scope
