Add a notes component

When I work on a participant I may record notes I would like to keep, for example if the individual has called for status, if their case worker provided details

Please create a notes component. It'll live on the individual profile page and have a save button.

Notes

• Add a notes value to participants table by editing init.sql (text datatype)
• Add create/update endpoints for adding/updating this field
• Add react notes component in the client for inclusion in the individual profile page
