Middleware zod-openapi uses https://github.com/asteasolutions/zod-to-openapi which has quite a few drawbacks, you can check comparison here: https://github.com/samchungy/zod-openapi/blob/master/docs/comparisons.md#asteasolutionszod-to-openapi.

I would suggest using https://github.com/asteasolutions/zod-to-openapi since it has more features and easier setup. For example it works with zod lazy without any additional workarounds.
