It's impossible to get the 25mm crossbar screw in along with the top left 45mm screw.

Still same issue. Could it be solved by just adding a few mm thickness to the adapter so the screws will clear each other?

It might be fixable that way, but the 40mm fan is suboptimal in several ways. The Owl Eyes dual inlet and CPAP are better choices without adding a lot of weight and losing build space.
