I still see "!" showing up next to the messages in the index display for flagged, even though I haven't configured "!" explicitly. I also see replied and unread showing up too.
Have I misunderstood the purpose of the nm_hidden_tags?

You mix two things:
maildir flags (used by regular mutt emails parser)
tags stored in notmuch DB
nm_hidden_tags are hidden for UI, but the flags are still valid and
used by mutt.
The code sync between tags and flags, but it's irrelevant in this context.
I have nm_hidden_tags set to "unread,draft,flagged,passed,replied,attachment".
