(Done in PR 719) Remove --enable-size-t=no and size_t=no configure options (switched to the "yes" by default in PR 428, issue 400)
(Done in PR 720) Remove all deprecated functions, macros, etc
(Done in PR 721) Remove OpenSSL 1.1.0 support (1.1.1 is the min version)
(Done in PR 722) Remove nt4 support
(Done in PR 724) Disable KeyValue and DEREncodedKeyValue by default
