BIPM: Indexing for SI Brochure

This ticket addresses remaining issues in BIPM SI brochure. When enhancements are implemented, we'll check outputs for BIPM files.

Index: I will complete implementation mid-week, expect to get this at earliest next week. Of course, this is also pending @manuel489 (I believe) actually adding index markup to the source asciidoc, once I have finalised it. (I am making up novel macros for this.)

Index functionality has been completed according to @opoudjis . This is the only remaining issue for the SI Brochure.
