Unify these https://docs.qhub.dev/en/latest/source/admin_guide/upgrade.html?highlight=upgrade and https://docs.qhub.dev/en/latest/source/admin_guide/breaking-upgrade.html

In conjunction with the "migration", we should strive to simplify the the "upgrade" docs (something similar to how JupyterHub handles this).
