Create Self Service User

Provide a brief overview of the enhancement or issue.
When you select a client then click on more and try to Create Self Service User the page is blank and shows just two checkboxes without labels.

Steps to Reproduce

Login to Mifos -> Go to Institution -> Select Clients -> Click on any client then select More on the Profile -> then select Create Self Service User

Expected Behaviour

It should give you an interface to create login credentials for the client to be able to consume /self APIs

Actual Behaviour

The page shows a blank interface with just two checkboxes.
