"/github-app/setup" not working on version 0.19.2

Overview of the Issue

In the version 0.19.2 the path"/github-app/setup" is not working. A blank screen appear instead of the correct.

Reproduction Steps

Follow the https://www.runatlantis.io/docs/access-credentials.html#github-app instructions on the version 0.19.2

Logs

No error logged.

Environment details

Tested on docker and standalone installation. I was able to access the path on the previous versions.

I think this is a dupe of #2141 which is fixed by #2142

Confirmed the issue, while quashed back in 0.18.2, is back in both 0.19.2 and 0.19.3 both.
