Acute hyponatremia (serum sodium <130 mEq/L): Asymptomatic: IV 50 mL bolus over at least 5 minutes; monitor serum sodium every 1-2 hrs to assess need for additional treatment. Symptomatic: IV 100 mL over 10 minutes; if symptoms persist, may repeat up to a total of 3 doses over a period of 30 minutes; monitor serum sodium hourly to assess need for repeat treatment. Chronic hyponatremia Asymptomatic or mild to moderate symptoms, severe hyponatremia (serum sodium <120 mEq/L): IV Initial: 15 to 30 mL/hr with frequent (eg, every 2 to 4 hrs) serum sodium monitoring; or alternatively, 1 mL/kg bolus (maximum bolus: 100 mL) over 10 minutes every 6 hrs as needed. Severe symptoms (eg, seizures, obtundation, coma, respiratory arrest) or known intracranial pathology: IV 100 mL over 10 minutes; if symptoms persist, may repeat up to a total of 3 doses over a period of 30 minutes, Alternatively, some experts recommend 150 mL over 20 minutes, up to a total of 2 doses while measuring serum sodium between infusions. Hyponatremia in neonate: 3%NaCl infusion rate about 0.1-1 ml/kg/hour (max infusion rate 1 ml/kg/hour) and a central line is preferred for administration for 3% NaCl. |
