I had a system app (let's call it App1) that I hid in Olauncher's special Hidden list. I later replaced App1 with a new app (App2), disabling App1 and also hiding App2. When I recently went through my Hidden list, I noticed both App1 and App2 were listed there. I wondered why App1 was still there as I'd disabled it from within Android. When I tried to open it it said something like "App not found." So I selected Show to un-hide it and it was removed from the Hidden list. When I went back to the main app list, it wasn't there either.
So it seems that, for some reason, the app wasn't removed from the Hidden list when it was disabled. Maybe something to look into...

That's because Olauncher maintains a local copy of list of hidden apps and they are not updated that frequently. I'll see if a check can be added for disabled app.
Disabled app didn't show up in main app list because that list is not saved locally. It comes directly from the system every time you open the app drawer.
