merge claims into encounters #1

Each claim is currently mapped to its own encounter. Two corrections need to be made:

Overlapping inpatient claims need to be merged to create a single inpatient encounter
Carrier claims that overlap with inpatient and outpatient claims need to be merged

This is necessary to get accurate statistics such as number of visits and total cost of the visit
