Enrollment criteria should be attached to a programme

Enrollment criteria should be persisted at each program-status level: e.g. program A - potential enrollment criteria, program A, active enrollment criteria.
Check if default enrollment criteria first, see if this is already implemented.

Criteria should be editable, even after a program has started. Changes to criteria should be tracked in changelog of each program.
