Table thead <th>: add scope="col" attribute #273

Hi,

Markdown tables are formatted with headers.
It would be nice if those headers (table thead th) had scope="col" attribute as in HTML5 spec.

It is reported by some linters (html-validate).

I looked at the code to make a PR but Go is not yet my friend :D
