Add PRT 4.3 requirement to text and update examples. #104

Change proposal that updates text to require PRT 4.2 and 4.3 to be populated with text and name of coding system.

Closes #93

I thought we agreed that we were going to populate both PRT-4.1 and PRT-4.2 with AUT, EQUIP, or RO. This is because in the TF volume 2, Appendix C.2 (CWE Data Type), the table shows that CWE.1 is RE, but CWE.2 is R.

An example would be EQUIP^EQUIP^HL70192.
