Refactor request

Many of us are finding the issue templates have fields that don't make sense to us. It would be great to make them easier to fill out, while including the necessary info.

Expected behavior

Align issue forms so they are more intuitive to fill out. (e.g., maybe we don't need this expected behavior box and instead an extra comments box that is optional might be helpful)
