completely disabled state persists over opensips restart

Summary

Calling the lb_status MI command for disabling (on demand) the destination. the destination will be completly disabled。

but once opensips restart, the completly disabed state will be lost.

i expect the completely disabled will keep persists over opensips restart

Solution

Add Probing mode 12 (completly disabled)

1. set the probing mode to 12
2. call lb_reload

@wangduanduan , there are actually 2 changes here in your PR

• a new probing mode "total disabling"
• ability to reload from DB without preserving the in-memory state of the destinations
  I find this indirect approach (for solving the main issue) a bit over the hand :( .
  The best approach will be to have something like this https://opensips.org/html/docs/modules/3.4.x/dispatcher.html#param_persistent_state, right ?
