fix: Change default question settings and minor ui fixes

• Make comment field in SORT not required
• Default consent is a single checkbox instead of radio
• Demographic question for age and time at organisation changed to check that it is an integer
• Removed question description from the consent, sort and demographic questions
• Fix required asterisk on form fields being on different line than label
• Display form field description
• Description for form fields now blank by default
• Resolves Survey Configuration Message Confirmation #117 - Return to Survey page after saving survey config and also displaying a success message.
