Include top textures, and update for MineClone 2 & 5

• Includes textures on top of blocks, to work better on servers with custom texture packs (see issue).
• Updates Less Dirt to support the new grass texture system that MineClone 2 & 5 switched to a while ago.
• Lists support of MeseCraft and MineClone 5 in the readme.
