Pass ts.Program to getCustomTransformers() (#889)

* Pass ts.Program to getCustomTransformers()

A lot of transformers requires a Program instance so we pass ts.Program
to getCustomTransformers() to be able to pass it on to transformers

* Update CHANGELOG.md
