If the altar for the Sword & Sorcery Offerings quest has disappeared run the console command sns_finishorders to finish that quest. Then just wait until the next season.
Passerby Cemetery has had map changes. You will need to reset everything on the Passerby Cemetery maps. Press v, select all options except for all maps, then press reset.
