[imaging uploader] Combine the two columns with number of MINC into one column only

Discussed at the imaging meeting of June 20th, 2019.

Instead of having two large columns with small numbers, combine the two columns "Number of MINC created" and "Number of MINC inserted" into one column with the proper information and links in the column:

"25 inserted (out of 26); 1 violation" with "25 inserted" linking to the imaging browser view session page for that upload and the "1 violation" linking to the MRI violation module.

Column name would be something like "MINC files"
