Loose ends with testing RefChef:

1. Spur - add primary (done), add annotation and index for primary ref (so add index and annotation separately to an existing ref)
2. add primary with a new yaml (done)
3. Add additional index, primary, and annotation
4. Try all of the above with just using appendding a reference

General note: these are the things the tests should look at.
