Removes in-line style and event handlers not compatible with header security policies. All event handlers now registered via main.js window event-handler on 'DOMContentLoaded'.

With this modification the X-ClientCert header is only set for proxy requests to the fhir app server if the user is authenticated with a client certificate and thus the SSL_CLIENT_CERT variable is not empty.
A generated mail address based on the domain in the iss (issuer) value and the sub (subject) value of the access token is now added to the current Practitioner object.

To support using Binary resources for HTML reports with inline css and java-script, adding a special case for Binary resources with content-type 'text/html'. Disabling inline css and java-script does not improve security since users that can create malicious Binary resource with HTML can also create malicious Binary resources with css or java-script.
TLS v1.2 re-enabled because the default curl implementation on Windows uses Schannel SSP for TLS and Schannel for Windows 10 does not support TLS v1.3
