Switched to function table for SCSI commands

moved defines/macros/etc to BlueSCSI.h
SCSI device state is now per device instead of global
Missing INQUIRY init added
SCSI-2 LUN handling (Fixes excess LUN's on SCSI-2)
Only allocate File obj for active SCSI ID's
