What this PR does / why we need it:

I dusted off the migrate tool and used it to move some old Loki data from one of my Raspberry Pi's using boltdb/filesystem to boltdb-shipper/S3 including a schema update.

This is the set of changes needed to make that work as well as some improvements to how the output and progress logging are handled.

Also updated the docs a little.
