consolidate all copyright notices into LICENSE file

@razorsedge Are you OK with moving all of the copyright boilerplate into the LICENSE file? It's completely fine if your not.

@jhoblitt I have tended to lean toward putting my name in files to which I may have munged things up... so that people know whom to blame, but I don't really care.

@razorsedge I'm just trying to cut down on the boilerplate in files since it requires some upkeep work. Doesn't git blame tell you who to, err, blame?

@jhoblitt I imagine that is does (I have not yet used it), but it relies upon having a .git directory. That data is not a part of a packaged Puppet module, so I always ask myself "what will the end user of my module see?" "Is knowlege of git/Github a requirement?"

I am fine with whatever you decide.
