Cyanide should play some sound to hint that someone is calling you and have a visual alert in the chat mode (as sound can be muted accidentally).
Have you tried jTox[1]? It seems to be actively developed.

jTox does not support voice/video calls. I have little use for pure text chats (and I need group calls too), so cyanide is the more advanced from those two. |

I see. So cyanide does kinda have audio calls, even though it's
outdated (and I'm surprised it still works). But group calls and
video calls are missing. They're not that hard to implement but it takes
effort to get it right. I can see myself fixing minor issues like this
but I'm not sure more features.
I think going forward it will be easier to add those features to jTox
since it seems more active and the code is in much better shape.
Perhaps an Android client like TRIfA works as well?

Outdated or not, it just works (unlike some trendy crap like viber/whatsapp).
This notification bug is the main user's problem.

TRIfA did not work for me (voice calls) even on a native Android tablet.
+ I prefer not to use any Android stuff in Sailfish.
