1. Add email address confirmation input …

   I accidentally removed this during the forms rewrite last year, which caused issues with dozens of attendees having invalid email addresses. This adds it back.
   
   Technically, the confirmation box appears when you claim your placeholder badge but it's not required to have any data in it. This is due to the core workflow with placeholder handling that we need to deal with later. Everything else should be working correctly.
