Adds DIC-4 as a dedicated example setup to demonstrate configuration options for a use case where HAPI is used in conjunction with CQL.
This is not set up as git LFS tracked file since we already exceeded our quota.

HAPI related JSON files have not been tracked with git LFS since we do not have a paid plan for using this feature.
