This reverts commit f54428f (nedbat#1347), which was a big regression in performance and file sizes when using dynamic_context, and is no longer needed for nedbat#972 as of 4cc3292 (nedbat#1538). Fixes nedbat#1586. Signed-off-by: Anders Kaseorg <andersk@mit.edu>

Most likely, your hook returns \[False]{.title-ref} for paths it does not want to ignore, which ends the processing and doesn't allow other plugins, including pytest itself, to ignore the path. We’ve been using this revert in Zulip without problems (zulip/zulip#25634), having previously been stuck on 6.4.2 for this regression.
