Summary

This PR addresses issues identified during a work session with the METS team, these include:

• Program states were not being initialised with a previous value which caused a minor bug with the "fixed-value" rendering
• The AFE supports capturing encounter dates configured with the ui-select-extended rendering
• Nested orders not rendering answer options correctly
• The patient identifier control breaks within the form-builder
