Added the following columns

  • payer (medical and pharmacy)
  • plan (medical, eligibility, pharmacy)
  • coinsurance_amount (medical and pharmacy)
  • copayment_amount (medical and pharmacy)
  • deductible_amount (medical and pharmacy)
