Currently to download an app (or get updates), you need to use AppStudio Desktop Edition.
Leaving the browsing of all available files (to download new) as a separate issue, this issue is to be able to get updates from ArcGIS online to an item that is already saved locally.

Reconsidering this issue, vscode behaves just like qt creator does now - you don't have the ability to get updates in qt creator, so it makes sense that this is not available (for now...) in vscode.
to get updates you need to use AppStudio. Higher priority would be the ability to choose Visual Studio Code as a preferred app editor from within AppStudio https://devtopia.esri.com/Melbourne/appstudio-desktop/issues/1068.
If in the future it is desired to be able to offer Visual Studio Code as an app creation environment without AppStudio desktop, then an update tool would be required.
