Contribution procedures

(all those procedures are needed because we need to build DCMTK outside of superbuild on appveyor due to time constraints)
- update the packaged version for appveyor build:
- commit the change to https://github.com/fedorov/DCMTK/tree/dcmqi (DCMTK/dcmqi branch)
- make sure the package of DCMTK is updated in the appveyor build https://ci.appveyor.com/project/fedorov/dcmtk
- update the hash in CMakeExternals/DCMTK.cmake: https://github.com/QIICR/dcmqi/blob/master/CMakeExternals/DCMTK.cmake#L28
- update the SlicerExecutionModel repository to use the updated DCMTK package: https://github.com/fedorov/SlicerExecutionModel/blob/dcmqi-311eff/appveyor.yml#L20
